Before we can start work on a project we need to have a written test request from you. It is preferred that you send the request sheet in with the samples but you can also send it by email to requests@coopertestinglabs.com, or fax it to us at (650) 213-8437. You have several options for requesting tests:
- If your company has a request sheet you can use it. At a minimum it should provide:
- Your company name, address and phone number,
- The project name and/or number and the project manager’s name,
- A list of all samples sent to us and tests to be performed on each sample.
- You can use our Test Request Sheet.
- You can request tests online using our online reporting system. Simply login and from the homepage click on the button entitled “Request New Tests” in the upper left-hand corner.
